May 31 (UPI) -- Jupiter, Fla. police released video Wednesday from Tiger Woods' Memorial Day DUI arrest.

Officer Matthew Palladino approached Woods' Mercedes-Benz from the passenger side. Officer Christopher Fandrey approached Woods' driver-side window.

The vehicle was running and had its brake lights illuminated. Woods' right turn signal was also flashing.

Woods told police he was coming from golfing in Los Angeles. He then stated that he did not know where he was. The police report states that Woods had "extremely slow and slurred, mumbled, confused" speech.

Woods eventually told officers he was coming from Jupiter, where he resides.

"Where you coming from man," Fandrey asks Woods in the video of the incident.

"LA," Woods responds.

"Where are you coming from right now," Fandrey asks Woods.

"I'm headed down to Orange County," Woods tells Fandrey.

"Do you know where you are at right now," Fandrey asks.

Woods responds that he does not know where he is. He tells Fandrey he hasn't been drinking. Woods says he did take prescription medications.

Palladino explains to Woods that he approached the vehicle because he was stopped "in the middle of the road."

Woods then took a field sobriety test, administered by Fandrey. He struggles to walk a straight line during the exercise.

He was given the walk and turn test, a one-leg stand test, finger to nose test and asked to recite the Romberg alphabet. Woods was able to recite the entire national anthem backwards, according to the report.

Fandrey also wrote that Woods had trouble tying his shoes. He eventually took them off.

Woods blew a .000 on his breath test.

The report stated that both of Woods' driver side tires were flat and the rims were damaged. The car also had damage on the driver's side front bumper and white scrapes and scuffs on the rear bumper, according to the report. The right rear taillight wasn't working, according to the report. TMZ also obtained photos of the damaged car.

The golfer, who is currently out with lingering back issues, was charged with DUI and taken to Palm Beach County Jail. He was booked at 7:18 a.m. and released on his own recognizance at 10:50 a.m. The incident occurred on Military Trail, south of Indian Creek Parkway.

Woods released a statement on Monday night where he said he took "full responsibility for his actions.

"I want the public to know that alcohol was not involved," Woods said in the statement on his website. "What happened was an unexpected reaction to prescribed medications. I didn't realize the mix of medications had affected me so strongly."

"I would like to apologize with all my heart to my family, friends and the fans. I expect more from myself too. I will do everything in my power to ensure this never happens again."

"I fully cooperated with law enforcement, and I would like to personally thank the representatives of the Jupiter Police Department and the Palm Beach County Sheriff's office for their professionalism."

Woods has a scheduled arraignment hearing on July 5.
