Arizona Motor Vehicles Department mistakenly issues licenses with incorrect photos

PHOENIX, June 25 (UPI) -- It's a case of mistaken identities...

Some Arizona residents have mistakenly been issued incorrect identification cards and driver's permits after a mistake by the Arizona Motor Vehicles Department.

When Jennifer Lohss looked at her temporary identification card and driver's permit, everything was fine. However, when she received the permanent copies in the mail, there was a problem.

"All of my information was correct. There was just somebody else's picture and somebody else's signature," Lohss told KPHO.

According to MVD officials, a new centralized system was recently put in place, but it's not clear if that is what is causing the issue. So far, there have been five reported cases.

"We immediately had a team assembled to assess what the issues might be. We don't believe that this is a systemic issue. We believe it's more individualized, and we need to find those nuances," said MVD Director Stacey Stanton
