"She's ready for her 30-day stint of therapy," a source told the magazine. "She's always put other people's needs ahead of her own, and her mental health suffered because of it."

Calvert and husband Jeremy Calvert appear in the Teen Mom 2 season 6 trailer released Monday, which foreshadows the couple's relationship problems. Jeremy Calvert reportedly filed for divorce in April.

Calvert and Jeremy Calvert married in April 2012, and share 2-year-old daughter Adalynn. The reality star is also mother to 5-year-old twin daughters Ali and Aleeah with ex-husband Corey Simms, who is seeking full custody of the girls.

Calvert came to fame on MTV series 16 and Pregnant, and went on to star on Teen Mom 2. Season 6 will premiere July 9 at 10 p.m. ET with fellow returning stars Jenelle Evans, Kailyn Lowry Marroquin and Chelsea Houska.
