Gary Goodman, state fair general manager, said the events leading up to the Sunday incident were unclear and no one aboard the Inverter -- a platform ride that fits 30 people into a car and then flips them upside down -- at the time of the accident reported seeing the accident.

The ride was closed for the remainder of the day Sunday while an investigation was carried out.
