Carolina Panthers offensive tackle Michael Oher was cited for allegedly assaulting an Uber driver in Nashville, Tenn., multiple media outlets reported Thursday.

The incident was reported on April 14, and Oher was charged with misdemeanor assault. He is scheduled to appear in court on Monday.

The 2009 Hollywood movie "The Blind Side'' was based on Oher's life story.

The Panthers noted that they are aware of the incident, but will not comment further.

According to the police report obtained by ESPN, the Uber driver told Nashville Metro police that he picked up Oher and four other people to go to a restaurant. The driver said an argument started after Oher told the driver he did not know where he was going.

When they reached the destination, "the victim then put his hands towards the suspect's face and the suspect pushed him down onto the ground." The driver then claimed "that the suspect then kicked him in the leg while he was on the ground."

The other passengers then restrained Oher.

Oher is currently in concussion protocol after missing the final 13 games of the 2016 season with a concussion.

Oher was the Panthers' starting left tackle until he was injured.
