CHICAGO -- Chicago Cubs shortstop Javier Baez left Sunday's game against the Atlanta Braves in the third inning with an injury.

Baez took a knee to the head during a headfirst slide on a stolen base attempt in the second inning. Baez, who was called out, remained on the ground after making contact with Braves shortstop Dansby Swanson and was checked out by Cubs trainer P.J. Mainville.

Baez took the field in the third inning before motioning to the Cubs dugout to alert manager Joe Maddon that he needed to leave the game. Mike Freeman replaced Baez at shortstop.
