Academy Award-winning actor Philip Seymour Hoffman was found dead on February 2 in his Manhattan apartment. He was 46. According to the Office of the Chief Medical Examiner of New York City Hoffman died of "accidental acute mixed-drug intoxication, including heroin, cocaine, benzodiazepines and amphetamine."
