The fourth count is that Suleman collected almost $10,000 in payments from Medi-Cal after failing to disclose $30,000 in income, the Los Angeles Times reported. She was already charged with receiving more than $16,000 in welfare in 2013.
Suleman, 38, became famous, or notorious, in 2009 when she gave birth to octuplets conceived by in vitro fertilization. She already had six children conceived the same way.
The octuplets turned five in January. Suleman was the second woman in the United States to give birth to that many babies, all surviving, at one time.
Suleman faces a significant jail sentence if she is convicted of two counts of aid by misrepresentation and perjury. She is scheduled for a hearing March 11.
The Los Angeles District Attorney's Office said she now owes the state now $26,286.29. Last month, Deputy District Attorney Bill King said similar cases are often settled without a conviction if the money is repaid.
