The runner, whose identity has not been reported, collapsed early during the race Sunday, The (Norfolk) Virginian-Pilot reported.
He was transported to a local hospital where he passed away Sunday afternoon.
"This is a tragic loss and our deepest sympathies go out to the runner's family and friends," Dr. P.Z. Pearce, the race's medical director, said in an email. "We have been in touch with the family and ask everyone to respect their request for privacy, and to keep them in your thoughts and prayers."
Virginia Beach EMS transported 12 other people to the hospital during the race for a variety of reasons.
Three other people have died during the Rock 'n' Roll Half Marathon in the past; Sean McCarthy, 27, of Richmond, in 2010, 23-year-old Erik Wellumson in 2009, and a man in his 50s in 2005
