Free agent guard O.J. Mayo has been dismissed and disqualified from the NBA for violating the league's anti-drug program, the league announced Friday. Under the terms of the NBA/NBPA Anti-Drug Program, Mayo is eligible to apply for reinstatement in two years.

Mayo, 28, previously served a 10-game suspension for performance-enhancing drugs in 2011.
Mayo has played the last three seasons with the Milwaukee Bucks. He played in 41 games last season, averaging 7.8 points and 2.9 assists.
In March, Mayo fractured his ankle when he accidentally tripped heading down the stairs of his home, causing him to miss the rest of the season.
Mayo was originally a No. 3 draft pick of Memphis, where he spent the first four seasons of his career. Mayo joined Dallas for one season before signing with Milwaukee.
In his career, Mayo has averaged 13.8 points, 2.9 assists and 3.1 rebounds.
The NBA, teams and the Players Association are prohibited from publicly disclosing information regarding the testing or treatment of any player under the Anti-Drug Program, other than to announce a player's suspension or dismissal from the league.
