The Jazz eliminated the Rockets in six games and will meet the Los Angeles Lakers in the Western Conference semifinals.
Utah overcame a 40-point effort by Tracy McGrady, who carried Houston into the playoffs despite the loss of center Yao Ming. The surprising Rockets produced the second-longest winning streak in NBA history before running out of energy in the playoffs.
Houston lost the first two games of the series at home.
Utah led by just four going into the second half, but the Jazz limited Houston to 11 points in the third period and owned a 20-point advantage going into the fourth quarter.
Williams scored 13 of his 25 points in the third and Mehmet Okur added 19 points and 14 rebounds.
McGrady took part in his seventh playoff series and has never been on the winning side. This marks the second straight season Utah has knocked the Rockets out of the playoffs.
The Jazz lost the first two games of the series last year and came back to win in seven.
