WILMINGTON, Del., Feb. 18 (UPI) -- A Delaware school district apologized after parents were mistakenly emailed a tongue-in-cheek "Hurt Feelings Report" form for "whiners."

The Brandywine School District apologized after parents of students at Wilmington's Lombardy Elementary were mistakenly sent the sarcastic form attached to an email about an upcoming "Exercise Your Brain Day" event.

The "Hurt Feelings Report" form purports "to assist whiners in documenting hurt feelings" and calls on the user to fill out information boxes including "Whiner's Name" and "Type of Whine Used."

"Did you require a 'tissue' for tears?" the form asks.

A parent who received the email told the Wilmington News Journal the form is "completely offensive and mocks children who are bullied in schools."

Brandywine School District spokeswoman Alexis Andrianopoulos said Wednesday a staff member accidentally attached the form to the email, which came from an address the school regularly uses to communicate with parents.

Andrianopoulos said the satirical form originated from "a source external to the school" and it was "not an official document of Lombardy Elementary or the Brandywine School District."

"This was an embarrassing mistake, but it was just that -- a mistake," Andrianopoulos said. "It should not have happened, and we apologize for the error."
