LOS ANGELES, Sept. 24 (UPI) -- Mordern Family star Sarah Hyland obtained a temporary restraining order against her ex-boyfriend Matt Prokop about one month after it was revealed the two ended their five-year relationship.

In court documents obtained by TMZ, Hyland recounted an instance in which Prokop came to her show's set, pinned her against a car in the parking lot, then began insulting and choking her.

"His grip was so tight that I could not breathe or speak. I was scared and in fear for my life," the actress says in the documents.

Hyland reportedly told Julie Bowen -- who plays her mother on the show -- that she knew she was in an abusive relationship and asked her for help to "peacefully end the relationship." She then asked Prokop to come over to her house one night, but he allegedly ran outside "screaming" when he saw Bowen and others in the house.

The actress said in the documents Prokop also threatened to set her house on fire and "relentlessly bombarded me with vile, threatening and emotionally disturbing texts and voice mails including his own suicide threats" after their breakup.

Hyland said Prokop entered a rehab facility in August, but left on Sept. 21 and that she received a call from the center advising her to get a restraining order against her ex. A judge granted her the temporary restraining order, which requires Prokop to stay at least 100 yards away from her and her dog.

The actress' lawyer confirmed the news to Us Weekly, adding that "the documents filed speak for themselves."
