LONDON, Oct. 5 (UPI) -- BBC One controller Peter Fincham has resigned amidst criticism he misrepresented film footage to make it look as if the queen stormed out of a photo session.
In July, a trailer for the documentary, "A Year with the Queen," was shown and Fincham told reporters she was seen "walking out in a huff" after a photographer suggested a wardrobe change.
However, the preview was edited out of sequence, the BBC said.
A report written by former BBC executive Will Wyatt criticized the BBC and the production company RDF Media for misjudgments and poor practice at a senior level.
The report also said the BBC delegated too much responsibility to RDF.
BBC director general Mark Thompson said, "Serious mistakes were made," and there was "misleading information about the queen (placed) into the public domain."
It is unclear whether the documentary will air.
RDF's chief creative officer, Stephen Lambert, has also resigned.
"I have had the tremendous privilege of leading BBC One through a period where the channel has undergone creative renewal," Fincham said in a letter. "Whilst I leave the channel with great sadness, I am tremendously proud of what I and my team have achieved."
